if the verb's voice beacame passive, then the order wouldnt make it that the IO came after the verb and before the DO:

this is obviously in the active voice since the subject is the doer of the action

my example:
A kiss was given to Shaira by Miguel.

so my example is in the passive voice beacause the Direct Object is the doer
of the action
and my example is in a totally diferent arrangement:
from STVDO to DOTVIOS

[like Sir Mojica said "there is always an exemption to the rule" and to their rule of "the IO should go after the verb and before the DO" well i think i know that exemption the position of the IO (and probably the DO also) depends on the voice of the verb. (and probably other things that can change this).

The object of the preposition is the noun that comes after the prepositional phrase(not in)
